Registration Procedure
Participants can register for the Meeting by the following methods:
• Register online
• Complete the Registration Form
Registrants are advised to read the registration information carefully before submission.
Payment
Payment of registration fees must be made in Hong Kong dollars by cheque payable to "The Hong Kong College of Anaesthesiologists" or by credit card (VISA/MASTER Card).
Details regarding the fees can be found in the online registration site or the downloadable registration form.
Cancellation Policy
Refunds for cancellation of registration will be made subject to the following deadline and administration charge:
On/before 30 September 2012 50% of the registration fee
After 30 September 2012 No refund
All cancellations must be made in writing to the Secretariat at asm2012@icc.com.hk and the refund will be made after the Meeting. The Secretariat will acknowledge receipt of your cancellation by email. Substitution is permitted as long as written notice is received before 7 November 2012.
Rules and Regulations
1. Each registrant should complete a separate registration form. Photocopy of the registration form is acceptable.
2. Registration form without payment will NOT be processed. Please do NOT send cash.
3. Secretariat will send a letter of confirmation by email upon receipt of your registration form and full payment. Kindly check the letter of confirmation. Any changes or alterations must be made in writing to the Secretariat.
4. Refund for cancellation of registration will be made and subject to the cancellation policy above. All cancellations must be made in writing to the Secretariat and the refund will be made after the Meeting.
5. The organizers reserve the right to amend the programme without prior notice. In the event of cancellation of the Meeting, the only liability of the organizers is to refund all the fees paid. Workshops may be cancelled due to insufficient enrolment. Refund will be made after the Meeting.
6. All enquiries, changes and cancellations should be made in writing to the Secretariat.
